The Forum Theatre was built in 1934. It was operated by the Schine Circuit in 1941. By 1943 it was listed as (Closed). It eventually became a J.C. Penny store, and was later converted into office space, which in 2024 is the Uptown Professional Building.

The building where this theatre was located still exists. At a later point in time, the Forum Theatre building was occupied by the J.C Penney store. There are multiple offices in the building currently.
Address is 19 East Main Street.
