The Cozy Theatre appears on the 1911 Sanborn, with a ‘From Plans’ note, indicating that it likely was not yet completed. It opened in 1912 by Elmer Hahn. The structure was a basic two-story wooden structure, likely quickly thrown up to take advantage of the movie craze. It was closed in 1915.
By 1922, there were better facilities for moviegoers, and this structure offered battery repair and charging. Despite its rather crude structure, the building can be seen on streetviews as recently as 2015 and last housed Short’s Furniture Store. The site is now a vacant lot.

That bit about the furniture store is wrong. The Shorts building was constructed sometime around 1922 as a garage. This building seems to be vacant on the older streetviews.
