This small neighborhood theatre appears on the 1915 Sanborn, in the northern half of a two-story brick commercial building. This area does not appear on the 1909 map, but the building appears to date to roughly 1910 or so.
This theatre likely could not draw enough clientele from a modest and rather sparse neighborhood, and the 1918 map shows this space as a feed store.
The building today is well kept, and this space was a restaurant which seems to be closed.

If 1343 N. Federal was formerly numbered as 1143 N. Main Street, then this must have been the house listed in the 1914-1915 American Motion Picture Directoryas the Kozy Theatre. I have yet to find the Kozy mentioned in the trade journals, and don’t really expect to. It is, however, listed in Polk’s 1914 Iowa Gazetteer.
Yes, Federal was Main. It’s crazy how many names changed. Delaware used to be Michigan, all the numbered streets dropped by 4 or 5 at one point, and before Federal was Main, it was Commercial. Thanks for the ID.
