This theater was located in the Park Manor neighborhood, on 69th Street between S. Prairie Avenue and S. Calumet Avenue. It opened in 1914, and could seat around 300.

The Park Manor Theatre closed in 1950 and was converted into a church.

Has this theatre actually been demolished? I’m only going by Google maps, but there’s a theatre-looking building at this address.
