Opened May 28, 1976 and operated until it was damaged in the Logan Valley Mall fire of December 16, 1994. It was later replaced with an eight-screen theater in another section of the mall.

Here is an aerial view of the mall right after the fire; the damaged Cinema IV is at the top center.

This opened on the same day as the Park Hills Cinemas on May 28th, 1976. Grand opening for both in photo section.
