This early Rapp & Rapp theater was announced in the September 25, 1912 Chicago Tribune. It was described as two stories high and of fireproof construction. By 1927, it had turned over to retail. In 1966, an apartment building rose on the site, which remains today.

The Chicago Historical Society has a database of Chicago Building Permits which lists a permit being issued November 9, 1912 to owner, North Shore Amusement Co. and architects C.W. & G.L. Rapp.
A Kimball theater organ size 2/8 was installed in the North Shore Theater in 1913. Note: Tubular Pneumatic Action.
Finally found an image of this theater. Look in the upper left of the photo, a dark buiding next to a 1-story white building. There’s a central arch with tile roofs to either side. 446 seats according to the 1914 Sanborn Fire Insurance Map. http://collections.carli.illinois.edu/u?/uic_idot,573
